Risk is an exposure to a possible bad outcome. Risk Services supports the campus by helping departments identify their risk exposures, recommending strategies to eliminate or reduce identified risks, and administering the campus's insurance programs to facilitate maximum recovery from loss events.

When reporting a work-related incident, injury or illness, please remember the following:

• If employee wishes to be seen by a doctor, complete the Authorization for Medical Treatment form (pdf). 
• Within 24 hours, complete the Incident Report Form (pdf) (Spanish).
• Within 5 days, submit the Supervisor's Incident Investigation Form (pdf).
• If an employee, contract worker or volunteer is seriously injured, Cal-OSHA must be notified within 8 hours. Get info.
