How to Obtain a UCSC Certificate of Self-Insurance
Summary: This page provides information about how to obtain a University of California Certificate of Self-Insurance.
When the University contracts with another party, the other party might request a Certificate of Insurance (COI). Risk Services issues Certificates of Self-Insurance in response to these requests upon receipt of an acceptable contract or agreement. Request a Certificate of Self-Insurance (certificate) for the type of contract or agreement negotiated:
Requests for contract review and/or certificates of insurance must be submitted at least 25 days in advance of the event or contract start (BUS-63).
Agreements that Include Payment to Another Party
If there is a payment to another party, then request a certificate using CruzBuy.
- Enter a CruzBuy requisition on a Services Form; see Using Forms to Submit Requisitions in CruzBuy
- If the supplier has presented you with a contract, then include an Internal Note within the CruzBuy requisition stating, "CERTIFICATE OF SELF-INSURANCE REQUESTED, CONTRACT ATTACHED" and attach a copy of the contract to the CruzBuy requisition
OR
If the supplier has not presented you with a contract, then include an Internal Note within the CruzBuy requisition stating, "CERTIFICATE OF SELF-INSURANCE REQUESTED, NO CONTRACT"
- If you have already submitted the CruzBuy requisition, contact Purchasing to request a certificate of insurance; see Purchasing Contact Information
- To check the status of your request use the History feature in CruzBuy: How to Review Status, Comments, History and Notes for a Requisition. If you have additional questions about the status of your request, contact Purchasing. Turn-around time depends on whether the contract is acceptable. Once an acceptable contract has been forwarded to Risk Services from Purchasing then the turn-around time is at least 10 business days.
Agreements with No payment Involved and Not Related to a Sponsored Research Agreement
If there is no payment, and the request for a COI is not related to a sponsored research agreement, then submit the contract to Risk Services with a request for a certificate. The request can be made by email, fax or inter-campus mail.
If the contract is consistent with the Standing Orders of the Regents, then a certificate will be issued. Turn-around time is at least 10 business days.
OR
If the contract needs to be revised, then Risk Services will direct the requestor to Business Contracts for assistance with the contract.
Contracts Related to a Sponsored Research Agreement
If a contract involves a sponsored research agreement, then submit the contract to Risk Services with a request for a certificate. The request can be made by email, fax or inter-campus mail.
If the contract is consistent with the Standing Orders of the Regents, then a certificate will be issued. Turn-around time is at least 10 business days.
OR
If the contract needs to be revised, then Risk Services will direct the requestor to the Office of Sponsored Contracts for assistance with the contract.
