UCSC Certificates of Insurance Summary: This page provides information about certificates of insurance. When the University contracts with another party, the other party often requests a "Certificate of Insurance". Risk Services issues Certificates of Self-Insurance in response to these requests after ensuring that the request is in response to an acceptable contract or agreement. Before we can issue a Certificate of Self-Insurance, Risk Services must
review a copy of the complete contract, including any attachments, for
language related to indemnity and insurance.
Turn-around time for a Certificate of Self-Insurance is 10 business days after receipt of an acceptable contract. Please fill out the Request for Certificate of Insurance form (MSWord) and return it to Risk Services with a copy of the contract and all attachments to the contract. You can return the information by inter-campus mail (mail-stop: Risk Services), by fax (831-459-3268) or electronically, to Jennifer Yeager.
