How to Obtain Medical Care if Injured on the Job
Summary: If you are injured or become ill as a result of your UCSC employment and need medical attention, please follow the following procedures.

Obtain medical authorization. |
- Notify your supervisor or other management employee in your unit that you have been injured on the job and need medical treatment.
- You will be given a signed UCSC Employee Authorization for Medical Treatment.
- You will need to present this authorization to receive treatment.

Obtain treatment. |
- For emergencies, call 9-1-1 for paramedic response and ambulance transport to Dominican Santa Cruz Hospital Emergency Department.
- For all non-emergency on-the-job injuries, Santa Cruz Occupational Medical Center at 610 Frederick Street in Santa Cruz is our designated facility, open weekdays from 8:00 am to 4:30 pm.
