COVID-19 Booster Clinic

The Faculty & Staff Wellness Program in the Office of Risk Services is partnering with Kaiser to provide a voluntary COVID-19 Booster Shot Clinic for faculty, staff, and students. You are eligible for a booster if it's been at least two months since receiving the J&J COVID-19 Vaccine or five months since your second Moderna or Pfizer dose. Both Pfizer and Moderna boosters are available. 

Wellness giveaways will be handed out at the event, and there will be a sign-up sheet for booster clinic participants to be entered into a raffle to win a Fitbit donated by Kaiser! 

We have limited capacity for appointments, so please sign up as soon as possible. If you are unable to make an appointment, you can obtain a booster shot from a local provider or pharmacy such as CVS, Walgreens, or Safeway. Instructions for submitting proof of obtaining a booster shot can be found here.

How to register for an appointment: 

To register for your appointment, click here. For your appointment, be sure to bring the filled out consent form (English form or Spanish form), your medical and prescription card, and your CDC vaccine card. Please note that space is limited and is by appointment only on a first come, first serve basis.

Preparing for your appointment:

Prior to arriving on campus for your booster shot appointment, please make sure you complete the Qualtrics Daily Symptom Screening Survey on the day of your appointment. To fill out this survey, follow the directions located here. Once cleared, you can come to campus. Masks will be required at all times, as well as six feet physical distancing.
